Kidney Care of Oregon is committed to the goals of better care, increased access, and reducing the overall spend on healthcare. Affordable and accessible healthcare is accomplished by working closely with our partners to make sure that the first question answered is always: How will this positively impact the patient? By asking this question, we are able to create care delivery models that are finely tuned to each patient based on their diagnoses. These care delivery models work to slow or reverse disease status.
There are programs with a range of designs dependent upon the needs of each patient. Some are designed for collaborating across specialties to align efforts both locally and regionally, while others focus on home care and management through robust tracking to maintain the patients’ commitment to meeting their health goals. All programs are reviewed and updated as the data supports changes to maximize the targeted patient outcomes. It is important for our team to foster an environment of shared decision making with our patients which includes education on lower cost alternative care methods.
